To create a Patient Portal account, visit ARDIG's website and click on the 'Sign Up' button. You'll be prompted to provide some basic information, such as your full name, date of birth, email address, and a secure password. After submitting the required information, you'll receive a confirmation email to activate your account.
Certain basic information fields are used to link to your patient records. Changing this information may lead to the inability to find your records. For this reason, some basic information fields are locked. To update any information in these fields, please contact us at planning@ardig.org.
